Care on the Move

Long-distance Personalised Care Transport

A new way of thinking

Typically, when a person needs assistance to move a long distance due to mobility or health needs they would be transported by a Patient Transport Service (PTS) ambulance crew. While a PTS crew is usually specifically trained in moving and handling of patients, and the use of ambulance-based equipment, they are unlikely to be trained to manage things like personal care, hydration, nutrition, and routine medication administration.

The Care on the Move concept considers that during a long distance journey it is likely that some sort of personal care, nutrition, hydration or medicines will be required. Care on the Move is different to Patient Transport because the person is unlikely to be a “patient”, but a Service User who needs to get from either their home or a care home, to another place of care a long way away. For the purposes of Care on the Move, these journeys are likely to be more than 3 hours.

Benefits of Care on the Move

Why should you choose Care on the Move instead of a traditional Patient Transport Service?

Dedicated, Professional Care for your loved one

During the journey, your loved one is looked after by a Care Professional, trained to provide personal care, continence care, hydration, nutrition, and routine medication administration. They are further trained to assist with moving and handling using ambulance specific equipment.

The journey is planned to include stops along the way to coincide with when these things need to be done, as well as safety breaks for the driver. When care needs two pairs of hands, the driver can step in to assist.

Person Centered Planning

The journey is planned around the care needs of your loved one rather than being treated as a logistics exercise.

Wherever possible this includes planning collection after breakfast and the morning care routine, and arriving at the destination outside of meal times, personal care times, and bed times.

We discuss this with the care providers at each end, so that you do not have to worry about getting the right information for us. We also take information from the exisiting care provider and family so that we can plan to make the journey as comfortable and seamless as possible.
